This boke hiȝte ypocras
on of the beste surgeons þat euer was
& galien his felawe & socrates
to þem þis booke beryn witnese
for all þey weryn felawes in fere
while þey lefde in erþe here
and þorough þe grace of hevyne kyng
þey practiseden medecynes to helpyn [ ]
praye we al to ihesu hevyne kyng
he yif here soule gode wonyng
